That means you design the interface for the software running on the site. May that be forums, CMS, blogs etc. A web developer also designs... but the meaning of the word "design" is rather different. Your design is the interface, while their design is the software structure. You have experienced this difference in our current project. You designed the interface of the site, while i designed & developed the software required for it. So basically, web designer and website designer.. really not much of a difference.
